**The above release forms must be signed by the student (if 18 or older) or parent/guardian (if under age 18) for participation in Go-Karts and/or Axe Throwing.

Project Grad needs chaperones for the event night! , as well as chaperones the night of the event. CLICK HERE to sign up.
Any parent interested in being a chaperone the night of the event must have background checks completed before May 1. The LSR7 Human Resources department is unable to complete background checks nearing the end of school, so please do not delay in getting information into SLC. Information on how to do this will be emailed to you after you sign up.
