Physical Form/PRIVIT/Insurance Enrollment

The following information is relevant for parents of any student that plans to participate in a sport or activity governed by the Missouri State High School Activities Association (MSHSAA). Parents of those students should familiarize themselves with the information below.

The MSHSAA has amended their Constitution and By-Laws with regards to sports physicals. In the past, a student’s physical exam was good for one calendar year from the time of the physical. Furthermore, a physical dated on or after February 1 was good for the remainder of that school year plus the following school year. The new policy from MSHSAA makes physical examinations good for two full calendar years from the date of the examination. This new policy is good for all physicals dated February 1, 2018 and later. As an example, if a physical was completed on February 10, 2018, then that physical would be honored until February 9, 2020. On February 10, 2020, the student would need to provide a new physical examination.

Lee’s Summit West will continue to utilize the PRIVIT system for the upload of your physical examination documentation. In addition, each year you will still need to sign off on the Commitment Form and the Parent Slideshow to complete your profile. If your MSHSAA Activity does not require a physical (Debate, Choir, Orchestra, Scholar Bowl) you will not need to use the PRIVIT system for the commitment form. If you are in one of those activities, then the form will be included in the packet of information given to you by the Sponsor/Director.
